ACU provides members with two (2) options for dental coverage through Humana: 

1. Humana PPO / Traditional Preferred Plan: (GA TRP U&C+) provides you with the choice of going to an In-Network PPO dentist (Nationwide network) or any dentist out-of-network! 

• $50 Individual Deductible / $150 Family Deductible for in-network providers
• $1,500 Annual maximum per covered person.  However, with the Extended maximum, Humana will continue to cover 30% of all covered charges after you hit your maximum UNLIMITED!
• 3 Routine cleanings per year
• Preventive services are covered at 100% In Network
• Basic Services are covered at 80% after deductible In Network
• Major services are covered at 50% after deductible In Network and Implants are Included.
• You can see any dentist!!! (Out of network dentists may balance bill for amounts above the allowed amount.  Humana uses 90th percentile of UCR.)

PPO Rates Guaranteed to 11/1/2025: Individual - $41.39; Member + Spouse - $82.79; Member + Child(ren) - $105.56; Member + Family - $146.95 

2. Humana HD DHMO / Prepaid C150 Plan: Must see in-network Dentist (Limited HD DHMO / Prepaid C150 network)

• No deductibles
• No annual maximums
• No exclusions for pre-existing conditions
• Covered services are based on a co-payment plan for in-network providers
• Routine preventive services are covered at 100% after a $5 office visit co-payment.
• In-network specialists receive a 25% discount, including cosmetic and orthodontic procedures.

DHMO Rates Guaranteed to 11/1/2025: Member - $17.88; Member + One - $32.66; Member + 2 or more $46.96

Humana, through its Specialty Benefits division, offers ACU members affordable, high-quality vision coverage tailored to their needs.

• $10 Exams Every 12 months
• $15 lens copay for eyeglass lenses
• $130 frame allowance every 24 months.
• $130 Allowance towards contact lenses instead of frames and eyeglass lenses every 12 months.

The plan is easy to use!

• Present your ID card at the time of your visit. You'll pay any co-payments at that time.
